Jonathan Hitzman, MD
Jonathan C. “Jon” Hitzman, MD began practicing family medicine in Pendleton in 1997. He received his Bachelor of Science in Biology, and Master of Science in Microbiology from the University of Illinois in 1981, & 1984. He received his MD degree from Rush Medical College, Rush University of Chicago in 1988. He completed his Family Medicine Residency program in Redding California in 1991, and has been a Board Certified Family Physician ever since, having to recertify every 7 to 10 years. He and his wife, Jacquie moved their 4 daughters, Jessica, Alysha, Erica, and Larissa to Pendleton, and they consider Pendleton their true home. Jon, and another physician started Pendleton Family Medicine in 1999, although that partner has since left Pendleton in 2008. Jon now provides outpatient management of preventative medicine services, and cares for a variety of acute and chronic disease conditions. To maintain his own good health, you may see him walking all around town, while he tries to get in his 10,000 steps on most days. In his scarce free time, he has remodeled most of his house, he enjoys numerous outdoor activities, and occasionally brews some tasty homebrew beer.
Danielle Addleman, PA-C
Danielle K. Addleman PA-C (Physician Assistant Certified) is a native of Pendleton. Danielle received her undergraduate degree in General Science at Oregon State University and her masters in Physician Assistant Studies from Oregon Health & Science University. Danielle moved back to Pendleton in September 2013 to start her career as a primary care medical provider at Pendleton Family Medicine. Danielle enjoys managing her own patient panel and has developed expertise in chronic disease management, including Diabetes, Hypertension, Hyperlipidemia, Kidney Disease and Heart Disease. Danielle also performs physicals, medicare wellness visits, well woman exams as well as family planning. When Danielle is not at work, you can find her at the local crossfit gym, at her family’s cabin, or at home spending time with her husband and son.
Jacqueline Herman, PA-C
Jacqueline Herman PA-C – Jacqueline Herman, PA-C was born and raised in Pendleton, Or. She received her undergraduate degree from Oregon State University. She attended Oregon Health and Science University, earning her masters in Physician Assistant Studies in 2014. Jacqueline moved back to Pendleton, Oregon in 2014 to start her career as a primary care provider at Pendleton Family Medicine. She is excited to be practicing in the community where she grew up. When Jacqueline is not working she enjoys spending time outdoors with her husband, son and extended family.
Chloe Norris, PA-C
Chloe Kay Norris PA-C is a native Alaskan. She completed her undergraduate degree in Biology from Humboldt State University. She attended the University of Washington, MEDEX Physician Assistant program and graduated in 2012. Since graduating she has practiced as an Urgent Care Provider at HealthPoint Community Health Clinic in Federal Way, WA before moving on to Emergency Medicine. She spent 2 1/2 years in Northern California in a busy ER before transferring to St. Anthony’s ER here in Pendleton, OR. She has since decided to go back into Family Medicine and has been with Pendleton Family Medicine since April 2017. When not working she enjoys spending time with her husband and 2 kids exploring Eastern Oregon!
